Our Executive Web Site package includes the design of a form to
collect information from visitors to your site, which is stored
in a Microsoft Access® database. The design of the form and the
database is included in the fee for the Executive Web Site.
The database can be downloaded any time as frequently as you like.
The form must be limited to 50 discrete pieces of data that can be
stored in a database field. A database field can be thought of as a
column of information in a spreadsheet.
If you choose this hosting package but wish to use more than one
form, we will be happy to add others for a one-time fee. Also, please
note that due to the technical complexity of setting up a custom
form and connecting to a database, there is a $100 fee per form each
time you wish to make modifications to a form that collects
